Referring your patients

Who to refer

Any patient with a chronic, progressive or terminal medical condition who:

• Feels overwhelmed, confused or fearful
• Is struggling to integrate his or her medical condition
• Needs psychological, emotional, social and/or spiritual support
• Experiences difficulties addressing practical concerns related to health issues
• Faces a change in status, condition or setting
• Repeatedly needs or will need in-patient hospital care
• Does not comply with your treatment plan
• Will enter or is in hospice care

I work with clients by phone (with local numbers in the US and Mexico), by Skype, and in person with offices in Puerto Vallarta.
